Two habits make the worksheet honest. Ask the people who DO the work, not only the managers who describe it: the gap between the two lists is often the whole findings section. And score pain truthfully rather than politely: the process someone rates five-out-of-five hated is worth three rated diplomatic twos, because hated processes get adopted-around, and adoption-around is where errors live.
